The Lux Dentistry Membership Plan Is Not Dental Insurance, But An In-Office Savings Plan Offered Exclusively By Lux Dentistry. This Plan Becomes Active On The Date Of Service (Dos) On Which Payment Is Made. The Plan Is Valid For Exactly One (1) Year From The Date Of Purchase. All Services Must Be Completed Within The Active Plan Period. Unused Services Do Not Roll Over, And Missed Or Canceled Appointments Cannot Be Redeemed After The Plan Expires, Even If Previously Scheduled. The Plan Is Non-Refundable, Non-Transferable, And Valid Only For The Enrolled Patient. This Plan Cannot Be Combined With Dental Insurance Or Any Other Discount Programs.
