How to Prepare for Your Visit

Proper preparation helps your first visit go smoothly. Bring the items below and review the quick lists.

Photo ID

Government-issued photo ID (e.g. driver’s license or passport).

Insurance Card

Current dental insurance card (and medical if requested).

Medical History

A summary of conditions, surgeries, or hospitalizations we should know about.

List of Medications

Prescriptions and over-the-counter drugs and supplements you take.

X-rays / Records

Recent dental records or imaging from another office, if available.

Dental Forms

Completed new-patient forms if we sent them to you in advance.

To-Do List

  • Eat a normal meal before your appointment
  • Brush and floss beforehand
  • Wear comfortable clothing
